Gynecomastia

What is it?
Gynecomastia (lipomastia, steatomastia, pseudoginekomastia (Latin:gynaecomastia)) – enlargement of the male nipple, as a result of glandular, fibrous and adipose tissue growth. Most often it presents as bilateral hypertrophy, rarely affects only one nipple.

Causes of gynecomastia and its types:
Juvenile gynecomastia (physiological) occurs during puberty and is caused by the action of hormones (increase in estradiol to testosterone ratio). In 60% of men, it disappears in adulthood. In other cases, it takes the form of adipose tissue hypertrophy (the most common – associated with various types of overweight), adipose-fibrous, or adipose-glandular. Gynecomastia has also been observed in men under the influence of cardiac drugs used for reducing blood pressure, but also in kidney and liver diseases.

Removal of gynecomastia:
After excluding the hormonal causes and systemic diseases, patient should undergo ultrasound diagnostics to determine the type of tissue in the behind the nipple region. If we are dealing with adipose tissue or adipose- fibrous tissue, the liposuction of this area is performed (recommended method is Vaser-Lipo due to its high effectiveness). The treatment lasts about 40 minutes. and there are three 4- millimeter scars left behind. In the presence of the glandular tissue hypertrophy, an arcuate incision, 1 to 2 cm in length, is made at the lower pole of the nipple and the gland is removed. There is just a barely visible scar left after this procedure. The removed glands are examined histopathologically. The methods are combined if necessary, i.e. in mixed gynecomastia, or adipose and glandular.
